Building Therefore I'm's first booking system

A 15-year clinic group had 40,000+ consultations and no way to book one online. That became a self-serve booking flow and zero self-serve bookings turned into a 39% booking-to-visit rate across 241 appointments in just a month of launch.

Context

A crowded industry where trust is the differentiator

Therefore I'm is an aesthetic cosmetology brand based out of Kerala, treating concerns through doctor-led care. The Clinic has been building trust offline for 15 years. Started as Nilufer Clinic in 2010 became Therefore I'm in 2021, added a product line in 2022 and now runs 6+ clinics across Kerala.

India's aesthetic cosmetology market is accelerating quickly.

Consumers are moving from cosmetic products toward clinical expertise. As the market grows, so does the number of providers. The differentiator is no longer procedure availability. It's Trust.

But the Therefore I'm website had stopped fulfilling its purpose.

Challenge

A clinic with no way to book appointments online

More than 40,000 of the clinic's consultations had been booked the same way: a phone call, routed manually through the clinic's CRM team. There was no way for a client to book anything themselves.

Research started with these conversations: on-site observations at the clinic, interviews with the CRM team, doctors and clients and a client survey with a small incentive attached to encourage honest answers.

Talking to clients directly and mapping the client journey surfaced the real issues.

  • Clients weren't hesitant about spending money. They were hesitant when they didn't understand what they were spending it on.

  • Prospective clients struggled to see where they fit since not everyone arrives already knowing what they need.

  • A single way to make contact created resistance for anyone not ready to commit to a full booking yet.

Solution

What we built

Homepage, concern-first navigation - Discovery starts with what's bothering someone, not what it's called.

Client Journey Testimonials - A client talking about their treament experience because trust comes from specificity, not generic testimonials.

Booking Flow - The self-serve path that never existed. Three steps: who you are, type and time, confirm.

Get a Call Back - A lower-friction way in. For clients not ready to book, just to talk.

Key Decisions

What got decided, and what didn't get built

Two sites instead of one hybrid

A hybrid site would have unified SEO and login, but mixing booking with shopping breaks navigation and creates choice paralysis. Different audience, different intent meant Phase 1 shipped as two sites instead of one.

The website built around concerns, not treatments.

Not everyone arrives knowing a treatment name. Most only know what's bothering them. Two ways into every treatment: through a concern, or directly through a department's grid, for people who already know what they want.

The build was scoped to match the team's actual capacity.

Without a dedicated front-end developer, ambitious ideas were cut down to what could realistically be built and maintained. Prioritization ran on impact against feasibility: showcasing real treatment outcomes was high-impact but was deferred, since it needed a photo and consent pipeline that didn't exist yet.

Impact

Since launch, via the website

Set against prior 40,000+ consultations and zero self-serve bookings, this is the first time Therefore I'm has had a functioning digital entry point.

Reflection

Phase 1 gets a client to the clinic. Phase 2 is already planned: helping clients see their treatment progress, keeping information consistent after the first consultation, and helping people follow post-treatment care properly.

Research took a lot of time because we wanted to actually understand what people were facing, not jump to solutions. It served a bigger purpose too, informing the brand as a whole and helping us figure out if phase 2 was even needed. Once the direction was set, AI helped us move much faster than we could have alone.

We also learned when to stop, making full use of the resources we had instead of chasing every idea. Some ideas needed real development, so we learned how Claude Code works and how to connect it with Webflow, enough to understand how a codebase works and edit basic things.
